Delivery area. We currently ship to clients in the contiguous US, Alaska, Hawaii, and Canada. If you live outside of our delivery area, please join our mailing list for future distribution updates.
Rates. We offer flat rate shipping as weight/dimensions vary due to the custom nature of our product. Rates are as follows (all in USD):

Canada Area 1 includes Alberta, British Columbia, Manitoba, New Brunswick, Newfoundland and Labrador, Nova Scotia, Ontario, Prince Edward Island, Quebec, and Saskatchewan. Canada Area 2 includes all other provinces.
These rates may change in the future due to carrier rate changes.
How we deliver our forms. Our forms ship Ground or UPS Standard International via UPS in one box which contains the bodice, stand base, stand pole, and accessories kit. To ensure safe delivery, we insure each form shipment and require a signature. (See this page to read how UPS is addressing safety right now.) Our scanning kits are delivered via UPS and USPS and do not require signature.
Insurance. IMPORTANT - PLEASE READ BEFORE PURCHASING! Your form shipping fee includes insurance for replacement in case of damage or loss during shipment. In order to make a claim on your behalf and replace any damaged/missing parts without any cost to you, you must take photos of the unopened damaged box as well as more photos of the damage inside as you open it up and email these photos to us within 7 days. Without receiving this photo documentation within 7 days, we won't be able to file a claim and any replacement costs including shipping will be your responsibility.
Local pickup. Although we really love meeting our clients in person, we’re not able to offer this service at this time. We plan on resuming this service once the public health risk has subsided.
Duties and Taxes. If you’re receiving your form in Canada, it’s your responsibility to cover all duties and taxes and any associated broker fees upon arrival. We cannot replace forms that can’t clear customs because of non-payment of these fees. Please contact us before ordering if you have any questions.
